Few debates in the concealed carry community are as heated as appendix carry versus strong-side hip carry. Both have passionate advocates. Both can work. But when you weigh comfort, real-world speed, and — most importantly — safety, strong-side carry has a compelling case that deserves an honest look.

Let's break it down.

The Basics: What Are We Comparing?

Appendix Inside-the-Waistband (AIWB) positions the firearm at the front of the body, typically between the 12 and 1:30 o'clock position. The muzzle points downward toward the femoral artery and groin area.

Strong-side hip carry positions the firearm at the 3 to 5 o'clock position on the dominant side, with the muzzle pointing downward toward the ground and away from the body's core.

That muzzle direction difference is the crux of the safety debate — and it matters more than most people acknowledge.

Comfort: Strong-Side Wins for Most People

Appendix carry advocates often cite comfort as a selling point, but that comfort is highly body-type dependent. For slender, athletic builds, AIWB can be comfortable. For anyone with a larger midsection, it frequently isn't — especially when seated. The pistol digs into the abdomen, and the grip can press uncomfortably against the stomach for hours at a time.

Strong-side carry, by contrast, works across a much wider range of body types. The firearm sits along the hip — a natural resting point — and with a quality holster and gun belt, most carriers forget it's there within a few weeks of consistent carry. Kidney carry (4:30 to 5 o'clock) is particularly comfortable for larger frames, as the gun tucks naturally behind the hip bone.

AIWB proponents frequently argue that appendix carry is faster. And in a sterile, standing, square-range draw, there's some truth to that — the hand has a shorter distance to travel. But real defensive encounters rarely happen standing still, squared up, with both hands free.

Research from organizations like the FBI and incident data compiled by PoliceOne consistently show that defensive encounters happen at close range, often involve physical contact, and frequently require one-handed operation. In a clinch or grapple, strong-side carry can actually be easier to protect and access than AIWB, where an attacker in front of you has proximity to your firearm.

Experienced strong-side carriers with quality gear and consistent training achieve draw times that are functionally equivalent to AIWB in real-world scenarios. The difference, measured in fractions of a second on a flat range, rarely translates to a meaningful advantage in the field.

Safety: The Case That Matters Most

This is where the conversation gets serious — and where strong-side carry has a clear, anatomical advantage.

With AIWB carry, the muzzle is pointed at the femoral artery, the groin, and the inner thigh. A negligent discharge in this position can be fatal within minutes. With strong-side carry, the muzzle is pointed at the ground or toward the outer thigh — still serious, but significantly less likely to be immediately life-threatening.

Negligent discharges during reholstering are the primary risk with any IWB carry method — and with AIWB, the consequences can be catastrophic.

  • 2018, Georgia: A concealed carrier suffered a serious self-inflicted gunshot wound to the inner thigh while reholstering an appendix-carried pistol. The injury required emergency surgery and resulted in significant blood loss. The incident was attributed to a shirt tail entering the trigger guard during reholstering — a known and documented risk with AIWB.

The most dangerous moment in any carry day isn't the draw — it's the reholster. Common causes of NDs during reholstering include:

  • A shirt tail, jacket hem, or drawstring entering the trigger guard
  • A finger not fully clearing the trigger guard before the gun enters the holster
  • A soft or collapsed holster that doesn't hold its shape

With strong-side carry, a reholstering ND is directed toward the outer thigh or ground. With AIWB, it's directed toward the femoral triangle. The difference in outcome potential is significant.

The Strong-Side Setup That Works

If you're going to carry strong-side — and we think you should seriously consider it — here's what makes the difference:

  • A rigid, trigger-covering holster — Kydex or hard polymer that holds its shape during reholstering. Vedder, Safariland, and CrossBreed all make excellent options.
  • A quality gun belt — A purpose-built belt with a rigid core prevents sag and keeps the holster in position all day. This is the single most impactful upgrade most carriers can make.
  • Consistent carry position — Pick your position (3, 4, or 4:30 o'clock) and stick with it so your draw stroke becomes automatic.
  • The right cover garment — An untucked heavyweight tee, a flannel, or a light jacket. Length and structure matter more than bulk.

The Bottom Line

Appendix carry works for some people, and there are skilled, disciplined carriers who use it safely. But when you weigh comfort across body types, real-world draw speed, and — most critically — the consequences of a negligent discharge, strong-side carry is the more forgiving, more versatile, and for most people, safer choice.

The best carry position is the one you'll carry consistently, train with regularly, and execute safely under stress. For the majority of carriers, that's strong-side hip.
